Amphotericin B, USP (syn: Fungizone) is a polyene macrolide antifungal antibiotic. This bioactive natural product was isolated from soil collected from the Orinoco River (Venezuela) in 1955 and identified as Streptomyces nodosus. It is used to control contamination from fungi and certain protozoa. Amphotericin B also exhibits immunomodulatory properties. Mechanism of Action: Amphotericin B associates with membrane sterols (ergosterol in fungal cell membranes and cholesterol in mammalian cell membranes). It forms transmembrane pores within milliseconds, leading to leakage of essential ions and ultimately cell death. Acquired resistance is very low but can occur through reduced ergosterol content or structural changes in the cell wall. Amphotericin B also induces oxidative damage and stimulates immune responses, including activation of phagocytes and accumulation of nitric oxide and reactive oxygen intermediates (ROIs).
